As Editor in Chief of JAMA Psychiatry, I am pleased to highlight this randomized clinical trial, my pick for Clinical Trials of the Year, published in JAMA Psychiatry in 2025. Schizophrenia is a common and severe disorder associated with significant morbidity and lifespans shortened by as much as 15 years. This early mortality is primarily due to cardiometabolic disease. People with schizophrenia are more likely to be overweight, lead sedentary lifestyles, and use tobacco compared to the general population, and the antipsychotic medications used to treat the disorder worsen this picture with their liability for additional weight gain and sedation.
00:35 To date, we have not had particularly effective approaches to counteract the strong forces that lead our patients to have higher rates of diabetes, hypertension, and related risk factors leading to premature mortality. There is now hope that this picture is changing with the advent of GLP-1 agonists. of GLP-1 agonists. In the current study from Denmark, the investigators enrolled 154 patients with schizophrenia who were obese and had prediabetes.
00:59 They randomized these individuals to a GLP-1 agonist, semaglutide, or placebo in double-blind fashion and followed them for 30 weeks to compare glycosylated hemoglobin A1c and other outcomes between the 2 arms. This clinical trial had multiple methodological strengths successful recruitment from regional community clinics, as opposed to just academic centers; wide participant age range, promoting generalizability; and excellent retention and follow-up, despite a potentially challenging-to-study outpatient population.
01:30 Here's what they found. Semaglutide lowered HbA1c, reduced body weight by a mean value of 9.2 kg— that's about 20 lbs—and improved physical quality of life without exerting negative mental health consequences. These findings were both statistically and clinically significant. Why is this my pick? Because it's rigorous, timely, and takes on a major challenge in psychiatry.
01:54 It also has public health implications. GLP-1 agonists may meaningfully improve the cardiometabolic condition of people with schizophrenia, leading to better long-term health outcomes and potentially significant reduction in premature mortality. The message for clinicians and patients is clear about the utility of GLP-1 agonists, and the study will likely lead to reevaluation of guidelines for managing cardiometabolic risk in schizophrenia.
02:17 I’m delighted that our journal published this paper, and I hope you agree that it is a major contribution to the literature. Go to this article to learn more and visit JAMA+ Trials for more content like this.
